For Case Managers & Referral Partners
A credentialed RN companion for your discharge plan
Island Nurse Companion works with hospital case managers, discharge planners, social workers, care coordinators, and provider offices across Hawaiʻi. When a medically stable patient is cleared to fly but shouldn’t travel alone, we send a licensed Registered Nurse with them — one-on-one, door to door.
Credentials & documentation
Everything your vendor credentialing process is likely to ask for, provided directly to your facility:
- Hawaiʻi RN license verification — Current, unencumbered Registered Nurse licensure, verifiable through the Hawaiʻi DCCA Board of Nursing.
- Certificate of insurance — Medical malpractice and general liability coverage; a COI can be issued to your facility on request.
- Current certifications — BLS, ACLS, and NIH Stroke Scale, with copies provided on request.
- Immunization and TB records — Standard health records for facility access, available as your credentialing process requires.
- W-9 and business documentation — For vendor setup and accounts payable.
- HIPAA and confidentiality attestation — Patient information is handled to the same standard expected of any healthcare provider.

When we’re the right referral
- Patient is medically stable and cleared for commercial travel
- Inter-island transfer, mainland treatment travel, or a homecoming after hospitalization
- Cognitive impairment, mobility needs, or medication management that makes solo travel unsafe
- No family member is available or able to accompany the patient
When we’re not
Island Nurse Companion is not emergency medical transport, an ambulance, or an air ambulance, and does not replace provider orders, airline medical clearance, or a higher level of medical transport. Every patient is individually assessed before a case is accepted — and if a companion trip isn’t clinically appropriate, we will tell you plainly rather than accept the case.

How a referral works

Reach Violet directly at 808-315-0860 with the patient’s situation, destination, and target dates. Every inquiry is answered within one business day.
RN case review
An RN reviews medical history, stability, and itinerary to confirm the patient is appropriate for commercial travel with a companion — and says so directly if a higher level of transport is needed instead.
Coordination and handoff
We coordinate with the care team, airline, equipment providers, and ground transportation, and deliver a clear handoff to the receiving facility or caregiver.
Advance notice helps enormously. Travel planning may involve the patient’s care team, the airline, equipment providers, ground transportation, and the receiving facility — so the earlier in the discharge planning process we hear from you, the more options we have.
Coverage & Payment
What to tell the family about cost
Inter-island companion travel starts at $1,250 and Hawaiʻi-to-mainland starts at $4,250, varying with the patient’s needs, level of care, and travel arrangements. We are not able to accept Medicare. If Medicaid verifies coverage for transit and lodging, a discount can be applied toward the trip. Outside of that, services are private pay. We’ll provide a clear quote before anything is booked.
