Case Review

An RN reviews the patient’s medical history, travel itinerary, and needs to confirm the trip is appropriate for a companion — and that the patient is medically stable for commercial travel.

Coordination & Planning

We coordinate with the patient’s healthcare team as needed, along with airlines, equipment providers, transportation, and the receiving facility or caregiver.

Travel Day

Your RN companion is with the patient one-on-one from check-in through boarding, in flight, deplaning, and every step in between — monitoring, assisting, and managing medications as authorized.

Safe Arrival & Handoff

We support a smooth transition to the next destination and a clear handoff to the receiving caregiver or facility.

What exactly does an RN companion do during travel?

Your RN travels one-on-one with the patient from departure to arrival — assessment and monitoring within the RN’s scope of practice, medication support when authorized, help with mobility and travel logistics, and calm advocacy and communication with your caregivers and providers along the way.

Is this the same as an ambulance or air ambulance?

No. Island Nurse Companion is RN-led companion care for patients who are medically stable and cleared for commercial travel. It is not an ambulance, air ambulance, or emergency medical transport service, and doesn’t replace provider orders, airline medical clearance, or higher-level medical transport when that’s what’s actually needed.

Who typically requests a companion?

Families dealing with memory loss or dementia, patients who need help navigating the airport or a wheelchair and transfers, anyone whose vitals or medications need monitoring mid-trip, and families who simply can’t travel alongside their loved one themselves.

What areas do you serve?

Based in Hilo, serving inter-island travel across the Big Island, Oʻahu, Maui, and Kauaʻi, plus Hawaiʻi-to-mainland trips. Mainland destinations are considered case-by-case.

How far in advance should I book?

As early as possible — advance notice is strongly preferred, since planning can involve coordinating with the patient’s healthcare team, the airline, equipment providers, transportation, and the receiving facility or caregiver.

How do I request a companion?

Complete the intake form, or call, email, or message on Instagram or Facebook with a quick question first. Every request is reviewed personally before it’s accepted.

Will the same RN stay with us for the whole trip?

Yes — one-on-one, door to door, from check-in through boarding, the flight, deplaning, and safe arrival.

Can my companion help with a wheelchair or other mobility equipment?

Yes — coordinating equipment, boarding assistance, and safe transfers at every stop is part of the service.

Can the RN manage medications during travel?

Yes, when authorized and appropriate, within the RN’s scope of practice and in communication with the care team.

What if a case isn’t a good fit for a companion trip?

Every request is individually assessed. If a higher level of medical transport is actually needed, that gets said directly rather than accepting a case that isn’t safe.

What does this cost?

Cost depends on the trip’s distance, duration, and level of support needed — reach out for a personalized quote.

How do I pay?

At this time, we’re not able to accept Medicare. If Medicaid verifies coverage for transit and lodging, a discount can be applied toward your trip. Outside of that, all services are currently private pay.

Is my information kept private?

Yes — personal and medical information is treated with the same confidentiality and care expected from any healthcare provider.
